Loving Hands Preschool is owned and operated by Greensburg United Methodist Church. It is administered by a board of active church members under the advisement of the Preschool Director. Below are all of the Preschool forms that are required to enroll your child in our school or that may be needed during the school year. The majority of these forms can be found in the registration packet that was given to you when you first enrolled, but in case you need one, you can click on the form below and print it out from your own computer.
Enrollment Agreement 2026-2027
Our classes are assigned on a first come, first serve basis. This is the form that is required to secure your spot in one of our classrooms. Return this form to the Preschool Director along with the $100.00 registration fee to hold your place.
Parent Handbook 2026-2027
Contains all the information you will need to familiarize yourself with our policies and procedures.
Child Enrollment Form 2026-2027
This form is required by law and provides information on parents/guardians and doctors of the child, which would be required in case of an emergency. It must be completed and turned in before the first day of class.
Child Care Profile
This form enables the teachers to be more familiar with your child. It should be turned in before the first day of school to give the teachers time to review.
Parent Consent Form
This form gives the preschool permission to use your child’s picture for display boards and other advertisement uses, permission to visit different locations of the building, and permission for the use of Purell in the classroom.
Health Care Plan Documentation & Permission To Administer Medication This form should be completed if a condition exists that will require treatment or monitoring during preschool hours.
Automatic Funds Transfer Form
This form is for families who would like to pay their tuition electronically. Tuition payments will be automatically deducted from your bank account on the day tuition is due each month.
Immunization Exemption Form A copy of your child’s immunization record or the Immunization Exemption Form must be on file by the first day of school.
